The state’s most expensive hospitals are also among its smallest.

According to state data released last week, Nantucket Cottage Hospital and Martha’s Vineyard Hospital, both under the Mass General Brigham umbrella, have the highest “relative price” of any hospital in the state.

In 2019, Nantucket Cottage Hospital was the most expensive hospital in the state, with prices 87% higher than the statewide average, according to the Centers for Health Information and Analysis. Martha’s Vineyard Hospital was 65% higher than the average, according to the data.

The index looks at average prices paid by commercial insurers for different providers in the state, then ranks hospitals by how they compare to the statewide average. The data does not reveal absolute prices any single hospital charges for a given service.

While the island hospitals have the highest relative prices in the state, they care for a very small percentage of the state’s inpatient admissions, and account for just a fraction of the total payments issued to hospitals in Massachusetts. Nantucket and Martha’s Vineyard Hospitals counted for only 0.2% and 0.3% of total commercial payments made to hospitals, respectively.

Boston Children’s, the third most expensive hospital in the state, cares for a higher number of patients, resulting in a larger impact to the state’s total health care spending. According to the data, the hospital is nearly 60% more expensive than the statewide average, and receives 7.3% of total commercial hospital payments — the third highest percentage of any hospital in the state.

While the data gives a sense of where a hospital’s commercial reimbursements rank compared with its peers, the statewide relative-price data does not include government payers, such as Medicaid and Medicare. Therefore, even though some health systems rank among the most expensive in the state, those reimbursements may be a fraction of the kinds of reimbursements that hospital routinely receives. For example, Boston Medical Center is the 10th most expensive hospital when looking at commercial payments, but it received only 12.1% of its reimbursements from Blue Cross and other commercial insurers, compared with 74% of its payments from Medicare and Medicaid sources, according to quarterly financial data.
